LionStone Care in Kettering, OH is seeking a Part-Time MDS Coordinator (LPN) to lead MDS assessments and care planning for residents in a long-term care setting.

The role requires 2-3 years of clinical nursing experience, including MDS knowledge, strong organizational skills, and proficiency with EHR PCC systems. You will work with the interdisciplinary team to ensure compliant, high-quality resident care and accurate reimbursement reporting.

Qualifications

• Graduate from an accredited LPN program with active license.
• 2-3 years of clinical nursing experience, at least 1 year in long-term care or similar setting.
• Experience with MDS assessments is preferred.

Responsibilities

• Coordinate completion and submission of accurate MDS assessments in a timely, compliant manner.
• Collaborate with interdisciplinary team to develop and update individualized care plans based on MDS data.
• Review MDS data for accuracy and ensure timely submission to CMS/regulators.
• Communicate with residents and families about the MDS process and care plans.
• Support billing and finance teams to ensure correct reimbursement implications.
