Job summary

Waters of Tipton is seeking a licensed nurse (RN or LPN) to coordinate MDS assessments in a long-term care setting. You will compile required data, determine functional capacity, and support payer-related reimbursement under RUG-IV guidelines.

Responsibilities include OBRA, PPS, and MDS completion, care plan coordination with the IDT, timely documentation, and adherence to regulatory timelines. Collaborative leadership with administrators and clinicians is essential.

Qualifications

• RN or LPN license required.
• MDS knowledge and OBRA/PPS familiarity preferred.
• Experience with ROM data collection and RUG-IV is a plus.

Responsibilities

• Lead and complete OBRA, PPS, and MDS assessments.
• Coordinate and update care plans across disciplines.
• Schedule and document Care Plan conferences within timelines.
• Transmit required MDS data to CMS per guidelines.
• Maintain accurate ADL coding and ARD documentation.
