Avante at Ocala Skilled Nursing and Rehabilitation Center in Ocala, FL is seeking an MDS Coordinator to oversee and coordinate resident assessments, ensuring compliance with federal, state, and local regulations.

The role involves coordinating MDS submissions, updating care plans, working with the Interdisciplinary Team, and striving to maintain high Five-Star ratings while supporting residents and staff.

Qualifications

• Active, unencumbered Licensed Nurse (LPN/RN) in the state.
• Nursing Degree/Diploma from an accredited school.
• 2+ years of experience in a hospital, skilled nursing, or healthcare facility.

Responsibilities

• Conduct and coordinate the MDS assessments and care planning in compliance with all regulations.
• Ensure timely and accurate submission of MDS assessments to the State Repository per RAI Manual guidelines.
• Work with the Interdisciplinary Care Team to determine assessment review dates.
• Update resident care plans to reflect changes in health status or quarterly assessments.
• Monitor Quality Measures Reports to maintain High Five-Star Ratings.
• Educate and collaborate with nursing staff, residents, and families on care plans.
• Participate in surveys and inspections by regulatory agencies.
• Maintain confidentiality and uphold patient privacy.
