Avante at Ocala Skilled Nursing and Rehabilitation Center in Ocala, FL, is seeking an MDS Coordinator to oversee resident assessments and ensure regulatory compliance. You will collaborate with an interdisciplinary team to maintain high-quality care and support timely, accurate MDS submissions.

This role emphasizes ongoing education, teamwork, and a proactive approach to care planning, with a focus on maintaining excellent patient outcomes and survey readiness.

Qualifications

• Active, unencumbered licensed nurse in the state.
• Nursing degree or diploma from an accredited program.
• 2+ years in hospital, skilled nursing, or healthcare setting.
• Knowledge of nursing practices, procedures, regulatory guidelines.
• Leadership with ability to motivate and collaborate with teams.
• Strong organizational and critical thinking abilities.
• Compassionate, patient-focused approach toward residents.

Responsibilities

• Coordinate MDS assessments and care planning following regulations.
• Ensure timely, accurate submission of MDS to State Repository per guidelines.
• Collaborate with Interdisciplinary Care Team on review dates.
• Update care plans reflecting health status changes and quarterly reviews.
• Monitor Quality Measures with aim to sustain Five-Star ratings.
• Educate and partner with nursing staff, residents, families on care plans.
• Participate in surveys and regulatory inspections.
