Avante at Melbourne Skilled Nursing and Rehabilitation Center is seeking an MDS Coordinator to oversee resident assessments and ensure regulatory compliance. The role offers a $3,000 sign-on bonus and a supportive, collaborative work environment.

Ideal candidates hold an active LPN/RN license in Florida and a nursing degree. Expect 2+ years of healthcare experience, strong organizational and leadership skills, and dedication to compassionate patient care.

Qualifications

• Active, unencumbered Licensed Nurse (LPN/RN) in the state.
• Nursing degree/diploma from an accredited school.
• 2+ years of experience in a hospital, skilled nursing, or healthcare facility preferred.
• Strong knowledge of nursing practices, medical procedures, and regulatory guidelines.
• Leadership skills with the ability to motivate and collaborate with interdisciplinary teams.
• Excellent organizational and critical thinking abilities.
• Compassion, patience, and a positive attitude toward residents and team members.

Responsibilities

• Conduct and coordinate MDS assessments and care planning in compliance with regulations.
• Ensure timely submission of MDS assessments to State Repository per RAI guidelines.
• Collaborate with Interdisciplinary Care Team to set assessment review dates.
• Evaluate and update resident care plans reflecting health status changes.
• Monitor Quality Measures to maintain high Five-Star Ratings.
• Educate and collaborate with nursing staff, residents, and families on care plans.
• Participate in regulatory surveys and inspections.
• Maintain confidentiality and ensure patient privacy.
