The role involves coordinating MDS submissions and collaborating with the care team to optimize plans while maintaining high Five-Star ratings. We value leadership, clinical excellence, and compassionate patient care.

Qualifications

• Active, unencumbered nursing license in the state.
• 2+ years in hospital, skilled nursing, or healthcare setting preferred.
• Strong knowledge of nursing practices and regulatory guidelines.

Responsibilities

• Conduct and coordinate MDS assessments and care planning per regulations.
• Ensure timely submission of MDS to the State Repository per guidelines.
• Collaborate with Interdisciplinary Care Team on review dates and care plans.
• Update resident plans reflecting health changes and quarterly assessments.
• Monitor Quality Measures to maintain high Five-Star ratings.
