You will collaborate with the interdisciplinary team to maintain accurate MDS submissions and support a high standard of resident care. This role emphasizes attention to detail, regulatory knowledge, and communication with staff, residents, and families.

Qualifications

• Active, unencumbered state license as LPN or RN.
• Nursing degree or diploma from an accredited program.
• 2+ years of experience in hospital or skilled nursing setting.

Responsibilities

• Conduct and coordinate MDS assessments and care planning per regulations.
• Ensure timely submission of MDS to State Repository per RAI guidelines.
• Collaborate with Interdisciplinary Care Team to set review dates.
• Update resident care plans reflecting health status changes.
• Monitor Quality Measures and support Five-Star ratings.
• Educate nursing staff, residents, families on care plans.
• Participate in facility surveys and regulatory inspections.
• Maintain confidentiality and compliance with privacy policies.
