Job summary

Pine Tree Lodge Nursing Center is seeking an experienced MDS Coordinator to ensure accurate assessments, regulatory compliance, and optimal care planning for residents. This role requires reliability, strong organizational skills, and collaboration with nursing and other disciplines.

You will coordinate admissions, annual, quarterly, Medicare, and change-of-condition assessments, maintain MDS data accuracy, lead interdisciplinary meetings, and update care plans as residents’ status changes.

Qualifications

• Current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication abilities.
• Knowledge of nursing home regulations and compliance.
• Able to work independently and in a team.
• Ability to manage multiple priorities accurately and efficiently.

Responsibilities

• Coordinate resident assessments including admissions, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure timely communication with nursing and other departments.
• Maintain compliance with Federal and State MDS guidelines.
• Review MDS and care plan files for accuracy and thoroughness.
• Maintain expertise in MDS software for data entry and reporting.
• Lead interdisciplinary meetings for Medicaid and care plan coordination.
• Update care plans based on assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards.
• Stay current through professional development sessions.
• Oversee ancillary data reporting to corporate billing.
