Job summary

Monarch Healthcare Management is seeking an LPN to coordinate MDS processes, ensure timely, accurate completion and regulatory compliance. You will collaborate with the Interdisciplinary Team to support care plans and billing accuracy while assisting nursing staff in high-quality resident care.

The role focuses on MDS completion, PDPM reviews and ongoing Medicare-related activities, with emphasis on documentation and teamwork in Minnesota facilities.

Qualifications

  • Certification in CPR/emergency care for health care provider preferred.
  • Ability to communicate verbally and in writing when directing the RAI process.

POSITION SUMMARY

TheMDSCoordinatorisaccountableforediting,lockingand thetransmissionprocessof all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. He/she oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT), and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality ofcare.

QUALIFICATIONS
Education and Experience
  • Must be a Licensed Practical nurse (LPN) in the state ofMinnesota
  • Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health careprovider.
  • Must possess the ability to communicate verbally and in writing when directing theResident Assessment Instrument (RAI) process and when performing and assessing care forresidents.
  • Must be able to perform essential functions of the position with or without reasonable accommodation.
  • Must qualify for employment after criminal background study, per godliness of Minnesota Department of Humansservices
Preferred
  • Experience in healthcare, skilled nursing facility or assisted livingcommunity
  • Experience with MDS and Resident Assessment Instrument(RAI)

ESSENTIAL RESPONSIBILITIES AND DUTIES
  • Completes admission chart reviews. Selects and communicates MDS ARDs to IDT. Reviews/Inputs ICD-10 codes into Electronic Health Record and ranks diagnosis. Completes Diagnoses Sheet(s) for billing.
  • Completes MDS in Electronic Clinical System based on observationof the resident, record review, data collection worksheets, NAR data, and interview with residents, families, andstaff.
  • Oversees timely completion of theMDS.
  • Participates in Care Area Assessment (CAA) documentation based on regulatory requirements, MDS information obtained, and in accordance with professional standards ofpractice.
  • Works with Interdisciplinary Team (IDT) to help assure care plans are comprehensive and completed ontime.
  • SubmitscompletedMDSwithinspecifiedtime frames;monitorsvalidationreportsforerrorsorother problems; corrects errors or problems, asneeded.
  • Oversees completion of necessary paperwork including Medicare certification and recertification forms; required notice; demand bills (including required paperwork for medical assistance payor); clinical documentation to support Medicare coverage; Monitors for Medicare eligibility after resident is admitted, e.g. rehospitalization or 30-daywindow.
  • Oversees PDPM review processes.
  • Conducts weekly Interdisciplinary Team (IDT) Case Management meetings to aid in management of Medicare/Managed Care A/B usage and Case Mix Review.
  • Monitors Quality Measures and collaborates with Interdisciplinary Team (IDT) to aid with improvement. Attends monthly QAPI. Completes QAPI report for each assigned facility.
  • Performs monthly Triple Check Process for Medicare A and Managed Care A caseload.
  • Assists with the training of staff, as needed, to help improve clinical care and accuracy of MDScoding.

  • Arrives at work area on time and ready to begin work. Is able to prioritize and be organized and efficient when interacting andcommunication.
  • Follows dress code, wears nametag, and limits personal phonecalls.
  • Always maintains resident confidentiality. Understands the need for complete privacy of all health caredata.
  • Follows policy on absenteeism, tardiness and call-ins as outlined in Personnel PolicyManual.
  • Is courteous and cooperative with residents, families, visitors, supervisors andcoworkers.
  • Promotes a positive and professional image both in the facility and in thecommunity
  • Applies knowledge of current policies and procedures as outlined by Monarch Healthcare Management
FACILITY COMPLIANCE
  • Promotes a highly positive workplace centered on strong customer service, inclusivity and ensuring staff feel welcome andsupported
  • Provides Rehabilitation Center with current license and keepsupdated.
  • Attends continuing education/in-services pertinent to your department and designated by theADON.
  • Attends nurses meetings at the RehabilitationCenter
  • Attends all staff education of the Rehabilitation Center relating to safety, infection control and disease process.
  • Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs. Participates in inspections and audits asdesignated.
  • Maintains current knowledge of Minnesota Department of Healthregulations.
  • Maintains and applies knowledge of current policies and procedures as outlined by Monarch Healthcare Management
