Job summary

Twin Pines Nursing and Rehabilitation Center is seeking an experienced MDS Coordinator to oversee resident assessments and ensure compliance with federal and state guidelines. You will manage documentation, data entry in MDS software, and lead interdisciplinary meetings to optimize care plans and reporting.

The role requires strong organizational skills, attention to detail, and the ability to work collaboratively with multiple teams to improve resident outcomes.

Qualifications

• Holds a current Nursing License and is in good standing.
• Experience in geriatric nursing and resident-centered care.
• Demonstrates excellent assessment, documentation, and communication skills.
• Understands nursing home regulations and compliance requirements.
• Works well independently and in a team.
• Can manage multiple priorities with accuracy and efficiency.

Responsibilities

• Coordinate resident assessments: admission, annual, quarterly, Medicare, and change-of-condition with timely communication to nursing and other departments.
• Ensure regulatory compliance with Federal and State guidelines for MDS and related assessments.
• Review MDS and care plan files for accuracy, thoroughness, and organization.
• Maintain expertise in MDS software for correct data entry and reporting.
• Lead interdisciplinary meetings and coordinate Medicaid, ITTM, and care plan discussions.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership to support care improvement.
• Ensure timely transmission of MDS data per reporting standards.
• Engage in professional development to stay current with industry standards.
• Oversee ancillary data reporting to corporate billing for accuracy.
