Job summary

Twin Oaks Rehab and Nursing is seeking an experienced MDS Coordinator to join our leadership team. You will oversee the completion and submission of MDS assessments, drive accurate care planning, and ensure compliance with federal and state regulations across the facility.

This role offers a collaborative, team-focused environment with supportive leadership and opportunities to mentor nursing staff on MDS processes, RAI workflows, and reimbursement impact.

Qualifications

• Current RN license in Massachusetts (required).
• Experience as MDS Coordinator in a skilled nursing facility (preferred).
• Strong understanding of RAI process, PDPM, and Medicare/Medicaid regulations.
• Excellent organizational and communication skills.
• Ability to work collaboratively in a team setting.

Responsibilities

• Complete and submit MDS assessments accurately and on schedule.
• Coordinate and lead interdisciplinary care plan meetings.
• Ensure compliance with all federal and state MDS regulations.
• Review medical records and consult with nursing and other RN and non RN staff to ensure accuracy.
• Monitor RAI process and update assessments as needed.
• Participate in quality assurance initiatives.
• Work closely with billing and reimbursement teams to support accurate claims.
• Serve as a clinical resource and mentor to nursing staff related to MDS processes.
