Twin Oaks Rehabilitation and Nursing Center in Danvers, MA is seeking a dedicated MDS Coordinator to join our leadership team. You will oversee and manage the MDS completion, ensure compliance, and support care planning and reimbursement processes.

This role requires an active MA RN license, familiarity with RAI, PDPM, and federal/state regulations, with opportunity to mentor staff and collaborate across disciplines.

Qualifications

• Current RN license in Massachusetts (Required).
• Experience as an MDS Coordinator in a skilled nursing facility (Preferred).
• Strong understanding of RAI, PDPM, and Medicare/Medicaid regulations.

Responsibilities

• Complete and submit MDS assessments accurately and on schedule.
• Coordinate and lead interdisciplinary care plan meetings.
• Ensure compliance with federal and state MDS regulations.
• Review medical records with nursing staff to ensure accuracy.
• Monitor RAI process and update assessments as needed.
• Participate in quality assurance initiatives.
• Support accurate claims with billing and reimbursement teams.
• Mentor nursing staff on MDS processes.
