Pine Ridge Post Acute is seeking an organized MDS Coordinator to ensure high-quality, resident-centered care. You will coordinate MDS assessments, develop individualized care plans with physicians and the care team, and support regulatory compliance and documentation accuracy.

Responsibilities

• Coordinate and complete MDS assessments within timelines per regulations.
• Develop and update resident care plans with interdisciplinary team.
• Facilitate care conferences and promote team communication.
• Ensure accurate resident records and documentation for reimbursement.
• Monitor residents and coordinate follow-ups to meet care needs.
• Assist with admissions, transfers, and discharges.
