Job summary

Twin Oaks Rehabilitation and Nursing Center is seeking an experienced MDS Coordinator to oversee and manage the MDS process in accordance with federal and state regulations.

You will lead care planning meetings, ensure accurate assessments, and mentor nursing staff in MDS procedures while collaborating with billing to support compliant reimbursement.

Qualifications

• Current RN license in Massachusetts required.
• Experience as an MDS Coordinator in a skilled nursing facility preferred.
• Strong knowledge of RAI, PDPM, and Medicare/Medicaid regulations.
• Excellent organizational and communication skills.
• Ability to collaborate effectively in a team setting.

Responsibilities

• Complete and submit MDS assessments accurately and on schedule.
• Coordinate and lead interdisciplinary care plan meetings.
• Ensure compliance with all federal and state MDS regulations.
• Review medical records and consult with nursing and other staff to ensure accuracy.
• Monitor MDS-related processes and update assessments as needed.
• Participate in quality assurance initiatives.
• Collaborate with billing/reimbursement teams to support accurate claims.
• Serve as a clinical resource and mentor to nursing staff on MDS processes.
