Job summary

Twin Pines Nursing and Rehabilitation Center is seeking an MDS Coordinator to oversee resident assessments, ensure regulatory compliance, and support care planning. You will manage MDS data, coordinate with nursing and clinical teams, and maintain accurate documentation to drive quality reporting and outcomes.

The role emphasizes attention to detail, reliability, and collaborative care in a geriatric nursing setting. Full-time, on-site position with ongoing professional development.

Qualifications

• Must hold a current RN license in good standing.
• Experience with geriatric populations and resident-centered care.
• Strong assessment, documentation and communication abilities.

Responsibilities

• Coordinate Admissions, annual, quarterly, Medicare, and change-of-condition assessments with timely interdepartmental communication.
• Ensure compliance with Federal and State MDS guidelines and regulations.
• Review MDS and care plan files for accuracy and completeness.
• Maintain and apply MDS software expertise for data entry and reporting.
• Lead interdisciplinary meetings and update care plans based on assessments.
