Valley View Care Center is seeking a RN/LVN with MDS experience to manage the RAI and care management processes from admission to discharge. The role requires knowledge of Case-Mix, Medicare PPS, OBRA, and MDS 3.0, with collaboration across the interdisciplinary team to ensure compliant care planning and accurate data entry.

The incumbent will coordinate information systems operations and education for the clinical department under guidance from leadership, and may support floor-level nursing

Qualifications

• Knowledge of Case-Mix and Medicare PPS.
• Understanding OBRA regulations and MDS processes.
• Experience with MDS 3.0 and care planning.
• Frequent and accurate data entry into MDS systems.
• Ability to coordinate interdisciplinary team activities.

Responsabilités

• Collaborate with Interdisciplinary Team to assess resident needs.
• Schedule MDS assessments and care plan reviews per regulations.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Ensure compliance with RAI process documentation and coding.
• Maintain accurate MDS data in medical records and systems.
• Assist on the floor as needed.
