Job summary

Creative Solutions in Healthcare, Inc. is seeking an MDS Coordinator to ensure accurate assessments and compliant care planning for residents. The role requires reliability, strong organization, and collaboration with nursing and interdisciplinary teams.

You will oversee admissions and annual MDS assessments, monitor documentation, and participate in quality reporting and care plan updates. A current nursing license and geriatric care experience are essential.

Qualifications

• Active Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance.
• Independent and team-oriented work style.
• Ability to manage multiple priorities effectively.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
• Ensure regulatory compliance with Federal and State guidelines for MDS.
• Review MDS and care plan files for accuracy and completeness.
• Maintain proficiency with MDS software for data entry and reporting.
• Lead interdisciplinary meetings for care planning and communication.
• Update and revise care plans based on assessments.
