Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to support our Forest Park Nursing Center in Texas. The role covers the RAI process, care management from admission to discharge, and adherence to Federal, State, and Certification guidelines.

You will work with the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems, educate staff, and ensure timely MDS coding, care plans, and documentation.

Qualifications

• Licensed RN or LVN in TX.
• Minimum 2 years nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Medicare PPS and Medicaid reimbursement.
• Familiarity with the MDS 3.0 process and OBRA regulations.

Responsibilities

• Collaborate with the Interdisciplinary Team to plan MDS assessments and care plan reviews.
• Decide on completing additional MDS based on clinical criteria per RAI Manual.
• Coordinate daily stand up meetings to review patient care and assessment dates.
• Ensure compliance with federal and state MDS / RAI regulations.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS software accurately and timely.
• Code MDS accurately from records, interviews, and observations.
