Job summary

University Health & Rehabilitation Center in Miami, FL is seeking an experienced RN MDS Coordinator to oversee resident assessments and care planning in our skilled nursing and rehabilitation facility. You will coordinate timely MDS submissions, support Medicare/Medicaid requirements, and work with nursing, therapy, residents, and families to develop individualized care plans and safe discharge planning.

Qualifications

• Active RN license in the state of employment.
• Two years of clinical experience in long-term care.
• Proficiency in MDS 3.0 and knowledge of the RAI process.
• Strong organizational skills and knowledge of applicable state and federal regulations.

Responsibilities

• Coordinate accurate, timely completion and submission of MDS assessments, CAAs, and care plans.
• Manage assessment schedules and support Medicare, Medicaid, and managed care requirements.
• Collaborate with nursing, therapy, residents, and families to develop individualized care plans and support safe discharge planning.
• Participate in utilization review, Triple Check, and quality improvement activities.
• Educate team members on assessment processes and documentation requirements.
