Job summary

Sans Souci Rehabilitation & Nursing Center is seeking an MDS Coordinator RN to lead the MDS process and coordinate care pathways. The role requires active RN licensure and experience with MDS, CAAs, and care planning in a long-term care setting.

The position emphasizes collaboration with the IDT, physicians, families, and payers, ensuring timely submissions and compliance with federal and state regulations. Equal opportunity employer.

Qualifications

• Registered Nurse with current, active license in state of practice.
• Minimum two (2) years of clinical experience in a health care setting.
• Minimum of one (1) year of experience in a long term care setting.
• Prior experience as an MDS coordination accepted.
• RN candidates may have a training program available.

Responsibilities

• Oversees accurate completion of the Minimum Data Set (MDS) and related assessments.
• Acts as in-house Case Manager coordinating services with physicians, families, and payers.
• Communicates regulatory risks and track clinical trends affecting outcomes.
• Ensures timely electronic submission of MDS data to the state database.
