Job summary

Lake Wales Wellness & Rehabilitation Center in Lake Wales, FL is seeking an MDS Coordinator (RN or LPN) to manage the RAI/MDS process, coordinate comprehensive assessments, and ensure compliant documentation for residents.

Requirements include an active Florida RN license, 3 years nursing with supervisory experience, MDS training within 6 months of hire, and RAC certification preferred. PTO, medical/dental/vision, life insurance & 401(k), and a supportive team are offered.

Qualifications

• Active RN license for Florida
• Minimum 3 years nursing experience including supervisory responsibilities
• MDS training must be completed within 6 months of hire
• RAC Certification preferred

Responsibilities

• Attend weekly education meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process per state and federal guidelines.
• Accurately complete all MDS assessments and any supporting assessments or clinical documentation.
• Evaluate residents' care plans, audit medical records, and collaborate with the interdisciplinary team.
• Perform additional tasks as assigned by Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information in all forms.
