Job summary

Onyx Health Consulting is seeking an experienced RN MDS Coordinator to oversee resident assessments and care planning in a skilled nursing and rehabilitation facility. You will ensure accurate documentation to support appropriate reimbursement and high-quality resident care, while collaborating with an interdisciplinary team.

Responsibilities include coordinating MDS assessments and care plans, managing schedules, and participating in quality improvement efforts.

Qualifications

• Active RN license in the state of employment.
• Two years of clinical experience in long-term care.
• Prior experience as an MDS Coordinator.
• Knowledge of the RAI process.
