Job summary

Manchester Center for Rehabilitation and Healing is seeking an experienced MDS Coordinator to join our skilled nursing team. The ideal candidate will oversee the MDS, CAAs and care plans in accordance with federal/state regulations and coordinate the RAI process with the IDT, residents and families.

Requirements include a current RN license, at least two years of clinical experience, and prior MDS coordination in long-term care.

Qualifications

• Must have current, active RN license in the state of practice.
• Minimum two years of clinical experience in health care.
• Minimum one year in a long-term care setting.
• Prior experience in MDS coordination preferred.

Responsibilities

• Oversees accurate completion of the MDS, CAAs and care plans.
• Communicates regulatory risk and care trends to leadership.
• Ensures timely electronic submission of all MDS to the state database.
• Educates IDT on the RAI process and coordinates with residents and families.
• Participates in QA/QM and other committee activities.
