Job summary

Preferred Care Management in Maryland is seeking an experienced MDS Coordinator (RN) to oversee the RAI process, ensure timely MDS assessments, care planning, and regulatory compliance for residents.

The ideal candidate holds an active Maryland RN license, has at least 2 years of MDS experience in skilled nursing or long-term care, and is proficient with EMR systems, CMS guidelines, and reimbursement processes. This is a full-time on-site role.

Qualifications

• Active Maryland RN license.
• Minimum 2 years MDS experience in skilled nursing or long-term care.
• Strong knowledge of RAI, CMS guidelines and reimbursement.
• Experience with EMR systems.

Responsibilities

• Coordinate and complete MDS assessments per CMS guidelines.
• Ensure timely and accurate submission of MDS assessments.
• Develop and update individualized care plans with the interdisciplinary team.
• Monitor documentation for regulatory compliance.
• Participate in Medicare and Medicaid reimbursement processes.
• Conduct care plan meetings with residents and families.
• Educate and support nursing staff on MDS processes and documentation.
• Perform audits and quality assurance reviews related to MDS and care planning.
• Stay current with regulatory changes and best practices.
