Job summary

South Shore Rehabilitation seeks a dedicated MDS/Care Plan Coordinator to ensure accurate resident assessments and up-to-date care plans, aligned with provided care. This role communicates resident conditions to families and coordinates care plans while maintaining regulatory compliance.

The position requires IL LPN licensure, MDS experience, and strong organizational skills. AANAC certification is preferred. Join a team focused on compassionate, compliant resident care and staff development.

Qualifications

• Must possess current IL LPN license and experience with MDS.
• Experience with MDS and care planning processes.
• Working knowledge of EMR systems and nursing administration.

Responsibilities

• Oversee MDS assessments at admission, readmission, quarterly and changes.
• Develop monthly MDS and Care Plan conference schedules.
• Ensure timely completion and accuracy of MDS sections by departments.
