Job summary

Nadia Care is seeking a full-time Maternity Navigator to support expectant and new mothers in the Marion County, Indiana area (Greater Indianapolis). You will build trust-based relationships, educate families on pregnancy health, postpartum care, and connect them to needed resources.

Qualifications

• 3+ years of experience in community health, patient advocacy, or healthcare navigation

Responsibilities

• Meet with mothers virtually and in-person, educating them on critical health topics
• Educate members on birth positions and breathing techniques
• Work as part of a multidisciplinary care team, attending daily huddles to escalate clinical issues
• Maintain clear and accurate documentation of all care interactions
• Build trusting relationships with mothers, creating a safe space for conversations
• Serve as a connector to community resources to overcome barriers to care
• Be a voice for the community to help shape Nadia Care programs
• Attend administrative meetings and follow policies and procedures
