Job summary

Nadia Care is seeking a full-time Maternity Navigator to support expectant and new mothers in the Montgomery/Prince George’s County, Maryland area. You will bridge healthcare and the community, building trusted relationships and providing education on maternal health during home visits and virtual sessions.

The role emphasizes empathy, collaboration, and proactive care, with hybrid work and a monthly on-call rotation. Compensation ranges per hour and full benefits are provided.

Qualifications

• 3+ years in community health, patient advocacy, or healthcare navigation as a certified professional.
• Knowledge of prenatal and postpartum health education.
• Strong listening and communication with cultural sensitivity.
• Ability to manage multiple priorities in a fast-paced environment.
• Driver's license with reliable transportation.
• Access to home internet and device for telehealth.
• Proficiency with AthenaOne, Welkin, and MacOS.

Responsibilities

• Meet mothers virtually and in-person, educating on urgent maternal signs, preeclampsia, postpartum recovery.
• Educate on birth positions and breathing techniques.
• Work with multidisciplinary care team and attend daily huddles.
• Maintain clear, accurate documentation of all care interactions.
• Build trusting relationships and provide support via call or messaging between visits.
• Connect to community resources to overcome barriers to pregnancy health.
• Provide insights to shape Nadia Care programs based on experience.
• Attend administrative meetings and follow policies to uphold standards.
