Nadia Care is hiring a Maternity Navigator to serve expectant and new mothers in the Memphis area. This full-time hourly role focuses on bridging the gap between healthcare and families, delivering education, support, and resources at home, in person, and virtually.

The MN builds trusting relationships, supports care coordination, and helps women access housing, transportation, and other social needs essential for a healthy pregnancy.

Qualifications

• 3+ years of experience in community health, patient advocacy, or healthcare navigation as a Certified Perinatal Community Health Worker or CMA.

Responsibilities

• Meet with mothers virtually and in-person to educate on urgent maternal signs, preeclampsia, and postpartum recovery.
• Educate on birth positions and breathing techniques to increase confidence and preparedness.
• Work with multidisciplinary care team and attend daily huddles with Nadia specialists.
• Document all care interactions clearly for coordinated support.
• Build trusting relationships and provide support via call or messaging between visits.
• Connect families to community resources to overcome barriers like housing or transportation.
• Provide insights from mothers to help shape Nadia Care programs.
• Attend administrative meetings and follow policies to maintain high standards.
