Job summary

The WestCare Foundation, Inc. is seeking a Care Coordinator in Miami, FL to coordinate services for women and infants through Healthy Start.

You will educate on childbirth, breastfeeding, women\'s health, and parenting, and link clients to essential community resources. You will maintain up-to-date case records, manage a caseload of 40+ per month, collaborate with primary care providers, and participate in ongoing training to support maternal and child health in a fast-paced nonprofit

Qualifications

• Requires a four-year degree in human services or related field.
• 2 years public health experience.
• Bilingual preferred.
• Valid driver’s license and ability to obtain WestCare insurance.

Responsibilities

• Coordinate services for pregnant women and infants.
• Maintain client records and progress notes.
• Provide education on childbirth, breastfeeding, women\'s health.
• Develop relationships with healthcare providers.
• Manage a caseload of 40+ consumers monthly.
