Job summary

Kids Central is seeking a Family Care Coordinator in Gainesville, FL to provide case management for Healthy Start services. The role includes assisting families with access to community resources, maintaining client records, and coordinating referrals in line with program guidelines.

Qualifications include a Bachelor’s degree in a relevant field, 2 years of related experience preferred, and a bilingual English/Spanish ability.

Qualifications

• Bachelor’s degree in social sciences, Social Work, Human Services or a Health-Related field is required.
• 2 years of related experience preferred; home visits with similar population preferred.
• Bilingual English/Spanish highly preferred.

Responsibilities

• Perform case management for Healthy Start clients and families.
• Assist clients in accessing community services and referrals.
• Initiate tracking procedures for clients, families and providers for appointments and referrals.
• Maintain/update client files and the Healthy Start database.
