  • Two years of public health/community development experience is required.
  • Bilingual is preferred.
  • CPR and First Aid Certification and an annual Tuberculosis test.
  • Ability to utilize Microsoft Office Software Suite.
  • Pre-Employment Drug Testing and Criminal Records Checks as required.

  • Provide short-term psychosocial care, if qualifications are met.
  • Ensure client records are accurate and up to date.
  • Link clients to resources (WIC, food banks, housing, mental health, etc.).
  • Ensure referrals and closures are documented in the database.
  • Follow program guidelines for outreach and communication.

Position Summary

The Care Coordinator coordinates services for women and parents of infants who were screened for or referred to Healthy Start.

Essential Job Functions
  • Responds to Healthy Start referrals in a timely manner consistent with Healthy Start requirements.
  • Provide education and support in childbirth, breastfeeding, women's health, and parenting to pregnant women and families with children aged 0-3.
  • Explain Healthy Start services to consumers, identify risk factors, and encourage consumers to participate.
  • Develop cooperative working relationships with primary health care providers.
  • Provide care coordination and referrals.
  • Maintain updated progress notes of services, referrals, and accomplishments in consumer files in a timely manner as outlined by Healthy Start Standards and Guidelines.
  • Maintain a caseload of approximately 40+ consumers per month.

  • Provide short‑term psychosocial care, if qualifications are met.
  • Ensure that all client records are accurate, current and updated on a regular basis.
  • Provide referral information to educate and improve client knowledge of available community resources, and link clients to services that support the well‑being of women during pregnancy and/or childcare (e.g., WIC, food banks, housing, mental health services, etc.).
  • Ensure referrals, as well as closures, have been appropriately linked and documented within the database system and agency data function.
  • Ensure required efforts are made when attempting to reach clients, as stated in the program's guidelines.
  • Follow appropriate telephone etiquette when answering incoming calls, leaving messages, and taking messages accurately by verifying callers information.
  • Develop and maintain a comprehensive understanding of maternal/child health issues that are deemed risk factors during pregnancy and early childhood.
  • Generate necessary correspondence in compliance with program guidelines of expected communications with clients and providers.
  • Maintain established quantity and quality standards.
  • Maintain professional work documentation and time management skills.
  • Develop and maintain good working knowledge of the Well Family System and State of Florida Healthy Start Standards and Guidelines.
Qualifications

Education: Graduation from an accredited four‑year college with a degree in human services, social sciences, social work, nursing, health education, health planning, healthcare administration, or related field is required. Graduation from an accredited masters program with a degree in Social Work, Psychology or related field is preferred.

  • 2 years of public health/community development experience is required.
  • Bilingual is preferred.
  • Must have reliable means of transportation.
  • CPR Certification, First Aid Certification and an annual Tuberculosis test.
  • Ability to utilize Microsoft Office Software Suite.
  • Ability to provide an Affidavit of Good Moral Character.
  • Clearance in Pre‑Employment Drug Testing and Criminal Records Checks, as required.
