Responsibilities include home visits, record keeping, coordinating care, training family networks, and collaborating with service agencies to ensure quality, timely services for pregnant women and infants.

Qualifications

• Bachelor’s degree required
• Minimum 3 years of relevant experience
• Bilingual, Creole or Spanish preferred
• Learning agility to adapt to procedures and priorities
• Strong communication and teamwork skills

Responsibilities

• Conduct situational analyses and provide technical assistance as needed
• Collaborate with agencies and leadership to deliver services
