Qualifications

• Associate's degree in social work, counseling or nursing preferred; experience may substitute.
• One year of social work or related family clinic experience preferred.
• Excellent interpersonal and good communication skills are required.
• Must have reliable transportation and valid driver's license with insurance.
• Basic computer knowledge; familiarity with Word, Excel, Publisher and PowerPoint.

Responsibilities

• Provides casework and determines intervention strategy per Healthy Start protocols.
• Completes intake, risk assessments and maintains client records.
• Follow-ups on missed appointments and home visits.
• Collaborates with coordinators to enroll fathers and support male participation.
• Maintains written and oral communication with participants and community education efforts.
• Performs other duties as assigned.
