This role coordinates subspecialty appointments, leads community outreach, and acts as the primary liaison between clients and resources, ensuring seamless care. This full-time position requires an RN license (BLS/CPR) with 2–3 years of experience and an associate or bachelor’s degree preferred.

Qualifications

• ASSOCIATE DEGREE or Bachelors preferred.
• 2-3 YEARS experience.
• REGISTERED NURSE license required.

Responsibilities

• The Navigator for the SMMT Grant coordinates subspecialty appointments and community outreach.
• Acts as primary contact and liaison between clients and referring sources.
• Maintains communication with community resources and clients.
