Responsibilities

• Assess participants’ social, emotional, economic, and environmental needs, and develop appropriate intervention plans; home visits may be required.
• Conduct interviews, prioritize needs, and accurately document case activities and progress.
• Collaborate as a team‑oriented department with managers, other departments, agencies, and caregivers to ensure alignment with treatment plans and continuity of care.
• Connect participants and caregivers with community resources, supporting discharge planning and ongoing post‑care needs.

Minimum Requirements

• Licensure / Certification / Registration: Masters Level Social Worker.
• Education: Master's degree in Social Work required.
• Work Experience: 3 years of experience preferred.
• Preferred Credential(s): Drivers License.
