Job summary

Sweetwater Care in Chico, CA is seeking a licensed LVN with strong long-term care experience to join our nursing team. The role involves providing nursing care within scope, administering meds and treatments per policy, and collaborating with physicians and care staff to meet resident needs.

Candidates with SNF/LTC experience and familiarity with PointClickCare charting are preferred; license must be active in California. This is a full-time, on-site position.

Qualifications

• Licensed LVN in California required.
• Minimum 1 year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long term care experience.
• PointClickCare charting experience.

Responsibilities

• Provide nursing care within scope of practice per standards and orders.
• Participate in quality improvement and use infection control manual.
• Monitor patients and coordinate care using Quality Life Manual.
• Notify departments of patient changes (room changes, LOA, admissions, discharges).
• Conduct family and patient education.
• Administer medications timely and per facility policy.
• Administer treatments timely and per facility policy.
• Receive, transcribe, carry out physician orders as permitted.
• Attend rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
