Job summary

Sweetwater Care is seeking an LVN with leadership experience to oversee nursing duties within a skilled nursing facility in California. The role emphasizes adherence to infection control, quality improvement, and timely medication administration under physician orders and center policies.

Candidates should have California LVN licensure and prior nursing management or long-term care experience, including familiarity with PointClickCare charting.

Qualifications

• Licensed LVN in California.
• Minimum of one year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long term care experience.
• PointClickCare charting experience.

Responsibilities

• Provides nursing care within scope of practice following standards, orders, and policies.
• Demonstrates knowledge and use of infection control procedures.
• Participates in quality improvement and uses the Quality Life Manual for patient monitoring.
• Notifies departments of patient changes and coordinates care.
• Educates patients and families.
• Administers medications and treatments timely per policy.
• Receives, transcribes, and carries out physician orders when allowed.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
