Job summary

Sweetwater Care is seeking an LVN with nursing management experience for a leadership role in our California facility. The role focuses on delivering high-quality nursing care within scope, following physician orders and center policies.

You will work under established standards, participate in infection control, and collaborate with physicians and families to ensure timely care and accurate charting in Point Click Care.

Qualifications

• Currently licensed LVN in California; compact license accepted if working in a compact state.
• One-year nursing management experience preferred.
• Long term care experience; familiarity with facility operations and resident care standards.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Utilizes Quality Life Manual for patient identification, placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admissions, discharges).
• Conducts family and patient education.
• Administers medications and treatments timely per facility policy.
• Receives, transcribes and carries out physician orders per Nurse Practice Act where allowed.
• Makes rounds with physicians when needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
