Job summary

Sweetwater Care is seeking an LVN in California to provide nursing care within the facility's policies and standards. The role emphasizes infection control, patient monitoring, and coordinated care within a SNF/LTC setting.

The nurse will administer medications and treatments, educate patients and families, and assist in implementing quality improvement efforts as part of the care team.

Qualifications

• Currently licensed as LVN in California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long term care experience required.
• POINT CLICK CARE charting experience required.

Responsibilities

• Provides nursing care within scope of practice.
• Follows infection control and quality improvement protocols.
• Participates in identifying and monitoring patients in programs.
• Notifies departments of patient changes.
• Educates patients and families.
• Administers medications and treatments per policy.
• Receives and transcribes physician orders when allowed.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
