Job summary

Sweetwater Care is seeking an experienced LVN supervisor to oversee nursing operations in a long-term care setting. You will utilize the Infection Control Manual, participate in quality initiatives, and ensure timely medication and treatment administration according to policy.

Applicants should hold an active California LVN license (compact license acceptable), have 1 year of nursing management experience, and be proficient with Point Click Care charting.

Qualifications

• Active LVN license in California (or compact license if in a compact state).
• At least 1 year of nursing management experience.
• Long-term care experience.
• Experience with Point Click Care charting.

Responsibilities

• Follow the Infection Control Manual and apply standards.
• Participate in and support the Quality Improvement Program.
• Utilize Quality Life Manual for patient placement and monitoring.
• Notify departments of patient changes (rooms, LOA, admits, discharge).
• Conduct family and patient education sessions.
• Administer medications and treatments per facility policy.
• Receive, transcribe, and carry out physician orders where allowed.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
