Job summary

Sweetwater Care in California is seeking an experienced LVN Nurse Manager to oversee nursing services in a long-term care setting. The role requires administering medications and treatments, supervising staff, and ensuring compliance with practice standards and physician orders.

The successful candidate will participate in rounds with physicians, educate patients and families, and coordinate admissions and discharges per facility policy.

Qualifications

• Current LVN license in California.
• 1 year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• PointClickCare charting experience

Responsibilities

• Administers medications and treatments according to facility policy.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• M akes rounds with physicians when necessary.
• Conducts family and patient education.
• Demonstrates ability to receive, transcribe, and carry out physician orders as allowed by Nurse Practice Act.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
