Job summary

Sweetwater Care in California is seeking a licensed LVN with nursing management experience to lead clinical operations in a long-term care setting. You will supervise nursing staff, ensure quality care, and coordinate daily activities to meet patient needs.

You will administer medications and treatments, transcribe physician orders within the Nurse Practice Act, participate in quality improvement initiatives, and educate patients and families while coordinating rounds with physicians.

Qualifications

• Active LVN license in California; compact license preferred for cross-state work.
• Minimum 1 year nursing management experience preferred.
• Long-term care experience required.
• Proficient with PointClickCare charting.

Responsibilities

• Provide nursing care within scope and in accordance with policies and guidelines.
• Administer medications and treatments timely per facility policy.
• Receive and carry out physician orders as allowed by Nurse Practice Act.
• Conduct rounds with physicians as needed.
• Educate patients and families; notify departments of changes.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
