Job summary

Sweetwater Care in California seeks an LVN to provide nursing care within scope of practice, following physician orders and facility policies. The role supports infection control, quality improvement, and patient education while ensuring timely medication administration and physician order execution.

Prior LTC experience and management exposure are preferred. Strong teamwork and communication with other departments are required to coordinate patient changes, admissions, discharges, and care

Qualifications

• Licensed LVN in California.
• 1 year of nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long term care experience.

Responsibilities

• Provides nursing care within scope of practice to meet patient needs per policies and guidelines.
• Demonstrates knowledge and uses Infection Control Manual.
• Participates in Quality Improvement Program activities.
• Utilizes Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes and conducts family/patient education.
• Administers medications and treatments timely per facility policy.
• Receives and carries out physician orders as allowed by the Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
