Job summary

Sweetwater Care seeks an experienced LVN in California to step into nursing leadership within a SNF/LTC environment. The role emphasizes policy adherence and quality of care across the unit, coordinating with physicians and multidisciplinary teams.

The successful candidate will oversee medication administration, patient education, and timely treatments, while collaborating on rounds with physicians as needed and ensuring compliance with state and center guidelines.

Qualifications

• Currently licensed as LVN in California.
• One-year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long term care experience required.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Uses the Quality Life Manual for patient identification, placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admissions, discharge).
• Conducts family and patient education.
• Administers medications timely and according to policy.
• Administers treatments timely and according to policy.
• Receives, transcribes and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
