Job summary

Sweetwater Care in Pasadena, CA is seeking an experienced Licensed Vocational Nurse to oversee patient care within scope of practice and facility policies. This role requires collaborating with medical staff, adhering to infection control, and participating in quality improvement initiatives.

You will administer medications and treatments per policy, document orders, and educate families. The ideal candidate has California LVN licensure (compact license a plus), and at least one year of nursing

Qualifications

• Must hold California LVN license; compact license if working in a Compact state.
• At least 1 year nursing management experience preferred.
• Experience in long-term care.
• Proficiency with PointClickCare charting.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders, if allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
