Elevance Health is seeking a LTSS Service Coordinator Clinician who will operate mainly in the field, traveling to member sites within Virginia. You will assist an RN with assessments, care plans, and coordination of physical health, behavioral health, and social services needs.

The role requires licensure as LPN/LVN, LCSW/LMSW/LSW, or another RN-exempt license, with 2+ years of relevant experience and a willingness to travel. Hybrid/office attendance may be required by policy.

Formación

• Must have current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN as allowed by state law.
• Minimum 2 years of experience with individuals with chronic illnesses, co-morbidities, or disabilities in a Service Coordinator or similar role.
• Details may vary by state law and contract; state-specified certifications may be required.

Responsabilidades

• Assist the responsible RN in identifying high-risk members.
• Obtain clinical data as directed by the RN.
• Help identify members who may benefit from alternative level of care or waiver programs.
• Provide collected information to the RN for verification and care planning.
• Coordinate care for members with chronic illnesses and disabilities to ensure cost-effective use of health benefits.
• Apply clinical guidelines and disease knowledge within the scope of licensure, without requiring nursing judgment.

Educación

LPN/LVN license or Nursing Diploma/AS in Nursing or related field

LCSW/LMSW/LSW license (state)

Current unrestricted license in applicable state(s)
