Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician who will operate in the field, traveling to client sites and designated locations. The role supports the RN, participates in telephonic or face-to-face assessments, and helps coordinate care for members with chronic illnesses and special needs.

The position requires licensure such as LPN/LVN, LSW, LCSW, or LMSW (or RN alternative as allowed by state law) and a minimum of two years’ experience in care coordination or similar roles.

Qualifications

• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
• Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• State-specified certification may be required based on state law and/or contract.

Responsibilities

• Assist the responsible RN in identifying members for high risk complications and data collection.
• Assist in identifying members that would benefit from an alternative level of care or waiver programs.
• Coordinate care with the health team to ensure cost-effective utilization of benefits.
• Provide information to the RN for care plan development, monitoring, and revisions.
• Support coordination of care for members with chronic illnesses and disabilities as directed.
