Job summary

Elevance Health seeks a LTSS Service Coordinator Clinician to support field-based care coordination for members in Virginia, traveling to client sites as needed. The role works under an RN supervisor and contributes to comprehensive LTSS plans, including health, behavioral health, and social services coordination.

The position emphasizes data collection, assessment support, and collaboration with the health care team to optimize member outcomes within state law and contract requirements.

Qualifications

• Must have active LPN/LVN, LSW, LCSW, LMSW or RN license in applicable state(s).
• Minimum 2 years of experience with individuals with chronic illnesses, co-morbidities, or disabilities in a service coordination or similar role.
• Previous experience in LTSS or care coordination is preferred.

Responsibilities

• Assist the RN in identifying high-risk members and collecting clinical data.
• Assist the RN in recognizing members who may benefit from alternate levels of care or waivers.
• Provide collected information to the RN for care planning, monitoring, and evaluation.
• Coordinate care for members with chronic illnesses and disabilities to ensure cost-effective use of health benefits.
• Apply clinical guidelines in decision making while understanding disease processes.
