Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician to operate primarily in the field, traveling to member sites with occasional office attendance for meetings or training. Qualifications

• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) required.
• Minimum 2 years of experience working with individuals with chronic illnesses in a Service Coordinator or similar role.

Responsibilities

• Assist in identifying members with high risk complications and obtain clinical data as directed.
• Coordinate care for members with chronic illnesses and disabilities with the RN and health care team to ensure cost-effective utilization of benefits.
• Assist in identifying members who may benefit from alternative levels of care or waiver programs; support development and monitoring of care plans.
• Conduct or assist with telephonic or face-to-face assessments within licensure scope as required.
