Job summary

Responsibilities

• Assist the responsible RN in identifying members for high risk complications and data collection
• Assist in identifying members that would benefit from alternative level of care or waiver programs
• Provide information to the RN for development and monitoring of the member care plan
• Coordinate care for members with chronic illnesses to ensure cost-effective utilization of health benefits
